Manitoba Immunization Card App

Manitoba is now offering an immunization card for people who are fully immunized.
When you request your immunization card, a digital card that you can carry on your mobile device will be created automatically. If you request a physical card, it should arrive in the mail within two weeks. These cards will include your first and last names and a QR code. When scanned with the Manitoba Immunization Card app. the QR code will show that you are fully vaccinated.
To be eligible to request your immunization card:
you must have a Manitoba health card
you must have received all required doses of a vaccine
(that means for Pfizer, Moderna or AstraZeneca, you require two doses.)
you must wait 14 days after getting your last dose of vaccine
Register to create your account and enter the required information - name, birthdate, health card registration number and Personal Health Information Number (PHIN). If you wish to have a physical card mailed to the address on your Manitoba health card, be sure to check the box before you submit your request.
NOTE: if the address on your Manitoba health card is not your current mailing address, go to
www.gov.mb.ca/health/mhsip/forms.html to update your address. Once you receive your new Manitoba health card, you can request your immunization card.
Request Your Immunization Card
NOTE: At this time, an immunization card is only available for people with a Manitoba health card. To find out if you are eligible for a Manitoba health card, go to the Manitoba Health and Seniors Care website. Once you receive your Manitoba health card, you can request your immunization card online. If you are not eligible for a Manitoba health card, you may contact your local public health office and they can provide you with a copy of your vaccination record.
